PADI Medical Statement

Recreational scuba diving is a fun and exciting, but physically demanding activity. This Medical Statement is used to find out if you should be examined by a physician before participating in scuba instruction. Keep in mind that a YES answer does not necessarily disqualify you from diving. It simply means that there is a preexisting condition that may affect your safety and you must seek the advice of a physician before scuba diving. We do ask participants to take a moment to look over the questions beforehand, just to make sure they have all NO answers. If you do have a YES answer, we will need to have Page Two signed by your physician and held on file before we can take you diving.
